Five Years of Love, Care, and Community in West Texas Hospice

Five years ago, Bridge Hospice opened its doors with a mission: to bring exceptional end-of-life care to West Texas families. Founded by Tammie Ware, a woman with a heart for service and a vision for care that goes beyond expectations, Bridge Hospice has grown into more than just a care provider—it’s a trusted partner during life’s most tender moments.

West Texas Roots, Local Heart

Lubbock isn’t just where we work; it’s where our story began. Our roots in West Texas shape how we care for our patients and families. It’s a place of resilience, connection, and unshakable values, and these principles guide everything we do. Over the years, we’ve grown alongside the community, recently moving into a larger office on 34th Street in Lubbock. This new space reflects our growth and commitment to serving families with compassion, dignity, and respect.

What Sets Us Apart

At Bridge Hospice, we believe in providing more than medical services—we offer a lifeline of support, comfort, and love. Our programs and services are designed to make a meaningful impact:

  • Dave’s Wish: A nonprofit initiative open to all hospice patients within our community, this program fulfills special needs and wishes, bringing peace and joy during challenging times.

  • Valor Program: Honoring veterans by recognizing their service and celebrating their lives with dignity and respect.

  • Bridge Volunteers: Dedicated individuals who create moments of joy and connection for patients and their loved ones, serving as a cornerstone of our care model.

  • Bridge Port: Our very own medical equipment team ensures all patient needs are met—whether at home or in a living facility—offering reliability and ease for families during difficult times.

Celebrating Excellence
Our dedication to excellence has been recognized in remarkable ways. Two members of our team were honored with the prestigious Charley Wasson Hospice Advocate Award. CEO Tammie Ware and CNA Tina Wall were celebrated for their outstanding commitment to hospice care. These accolades reflect the passion and dedication of our entire team, whose focus is always on serving others with kindness and grace.

Expanding Our Reach

In 2024, we extended our reach to the Plainview community, broadening our ability to provide personal, compassionate care to even more families across West Texas. This expansion is a testament to the trust our patients and their families place in us. It’s also a reflection of the incredible team at Bridge Hospice—a group of individuals who treat each patient and family like their own.
